PART A
CAMPER PAYMENT 

Welcome, Please READ Carefully.

Camper registration must be completed in two sections: 

PART A - CAMPER PAYMENT

PART B - CAMPER MEDICAL

You will be required to complete PART A - CAMPER PAYMENT first. 

Once PART A is completed. You can then proceed to complete PART B - CAMPER MEDICAL via web link from the payment confirmation page upon submission. Do not close your browser windows. A pop-up or redirect may happen. If not, Please (check junk/spam folders) as a backup copy will be sent via email. 

PART A & PART B must be both completed right away, in order to secure your spot for Camp Met 2025. 

Please read all sections carefully, take your time and fill-out all fields on the form. Any fields left blank will result in a pop-up error message and a declined camper registration.

Again "PART A - CAMPER PAYMENT" is REQUIRED in order to complete "PART B - CAMPER MEDICAL" .

BOTH SECTIONS ARE REQUIRED TO SECURE YOUR CAMPER REGISTRATION FOR CAMP MET 2025.

*Failure to complete ANY of the PARTS will result in a declined registration and refunded payment. Thank you.

*Duplicate Registration Payments will be subject to CHARGES* and refunded at a later date.

Cancellation/Refund

Camper Registration Payment:

I understand and agree, to the amount charged to my credit card per/ camper registration.

All registration fees include: two-way chartered coach bus transportation to/from Toronto to Haliburton, camp t-shirt, camp accommodations, meals, ropes climbing course and water front boating activities and on-site staffing.

Cancellation/Refund:

If withdrawal from Camp Metamorphosis Ontario is required. A cancellation request must be submitted by email only to: [email protected] on or before July 1^st, 2025: The refund will be issued, less a 10% ($150) deposit/administrative charge, per camper. There will be no refunds after July 1^st,2025. Refunds are upon discretion of the Camp Director once the request is reviewed. 

At Camp Met:

There will be no reduction in, or refund of, camp fees for campers arriving late or leaving early for which they are registered.  There will be no reduction in, or refund of, camp fees for campers who are expelled from the camp for breaking the camp code of conduct or otherwise, or are withdrawn from the camp prior to the end of the period for which they are registered.
